Information will be required from each client for the purpose of consulation for treatment and recording treatments.
Personal contact information such as address and telephone number shall only be used for the purposes of communicating to arrange and confirm appointments and will not be shared with any third party without written consent from the client.
All information will be hand written and will be retained in a safe place. Clients can request a copy of the information and such a copy will be provided.
Sharing of such information with relevant health professionals, such as a GP, will only be undertaken with the written consent of the client.
All information will be treated as confidential and will not be shared with any other person or organisation without the client's consent.
Any information sent or received via email or text or other electronic message device may be saved but is password protected.
All records will be stored for at least 7 years under the Department of Health Guidelines.
